Returning patients only need to fill the "Health History and Consent Form" (as long as your last visit has not been more than a year apart).

Please fill out these forms and bring them with you. If you do not have time to do this the forms will be provided at the time of your arrival.

Remember to bring your insurance card and I.D. with you. If you do not have insurance,we have affordable self pay options available. We look forward to helping you when you need it without delay.
